TRiO Talent Search Field Trip Form

I hereby grant permission for my above mentioned son/daughter to participate in the field trip/event listed above. In consideration of my child’s participation, I assume all risks and hazards incidental to such participation and hereby release, absolve, indemnify, and agree to hold harmless the LULAC National Educational Service Centers (LNESC) Inc. and Austin Independent School District employees and its officers. Neither LNESC nor any said persons shall be held financially responsible for any injury, illness, or death incurred as a direct result of this activity.

We understand that the Austin Independent School District’s Code of Conduct applies to all LNESC Austin TRIO Talent Search field trips and Events. We understand that said student will conduct themselves with maturity, courtesy, and respect toward all parties participating in the field trip, including, but not limited to, classmates, chaperones, teachers, and others.

In the event that said student fails to conform to the rules established by the LNESC Staff in charge, etc., I agree to accept full responsibility to promptly pick up my child from the location.

In the event of an accident or injury requiring medical attention, I hereby grant permission to the supervising LNESC Staff, chaperones, and/or Teachers to attend to my son/daughter. If the injury warrants further medical attention, I expect every effort will be made to contact me to receive my specific authorization before action is taken. If efforts to contact me are unsuccessful, I grant permission for necessary medical treatment to be given. In addition, I hereby give my consent to the supervising LNESC Staff, chaperones, and/or Teachers to contact emergency medical services

Please provide facts concerning the student’s medical history including allergies, medications, and any physical impairment to which LNESC Staff should know.
